September 9, 2023 Went to Post Office to submit filled-out DS-11 with the following:
1. Original and photocopy of U.S. Citizenship Evidence (US Birth Certificate, Certificate of Citizenship, etc.)
2. Proof of Parental Relationship (US Birth Certificate, Certificate of Citizenship, etc.)
3. Original and photocopy of Parent's Present Identification (State ID, Driver's License, Valid Passport, etc.)
4. Parental Consent
5. Recent Photo (colored, full front view of the face and 2x2 inches in size)
4. Fees (Passport Book: Application Fee $100 + Execution Fee $35)

Tips:
• Go here to fill-up application online.
• Do NOT sign your child's application until instructed to do so by the acceptance agent.
• Photocopy of U.S. Citizenship Evidence must be legible, on Standard-size (8.5"x11") white paper, black and white and single-sided
• Photocopy of Parent's Identification must be on Standard-size (8.5"x11") white paper. Cannot be double-sided and in actual/enlarged size.
• It's best for both parent/guardians to go with the child in person to apply for the passport.
• Do not attach your photo to the form before submitting it to the acceptance agent. Some passport acceptance facilities provide photo services for an additional fee. Go here more about photo requirements.
