April 5, 2010 Abigail went to St. Luke's Medical Center Extension Clinic (SLMCEC) for medical with the following:
• Medical Fee of $213.35 converted to Php
• Original/Copy of the National Visa Center letter
• Original/Copy of Passport
• 2x2 photos
• X-ray films not older than 1 year
Abigail rode jeepney from Meycauayan to Monumento. LRT from Monumento to United Nations and taxt from United Nations to SLMCEC.
Abigail submitted copy of NVC letter, Passport and 2x2 photo to SLMCEC guard. Got number and Personal Data Sheet (PDS).
Abigail submitted accomplished PDS at the reception area for digital picture taking and data encoding.
Abigail paid Medical Fee at the cashier.
Abigail finished X-ray, Laboratory, Vital Signs and Medical Examination.
Abigail received an Appointment Stub for Immunization.
April 6, 2010 Abigail went back to SLMCEC for immunization with the following:

Abigail submitted Passport and Appointment Stub.
Abigail finished her Tuberculin Skin Test and Immunization.
Abigail surrendered her Passport and received a copy of DS-3032 and X-ray CD.
Abigail went to nearest BDO for Visa Fee payment with the following:
• Visa Fee of $131.00 converted to Php
• Copy of Appointment Confirmation Receipt
Abigail received the BDO Visa Payment Slip.
April 16, 2010 Abigail went to US Embassy for interview with the following:
• Copy of Appointment Confirmation Receipt
• Copy of BDO Visa Payment Slip
• Passport
• DS-156
• DS-157
• DS-156K
• Birth Certificate
• NBI Clearance
• I-129F Package
• Proof of ongoing relationship (i.e. mail, e-mail transcript, phone bills, money transfer receipts)
• Certificate of No Marriage Record
• I-134 and supporting documents (i.e. Employer Letter, Pay Checks, W2, Form 1040, Bank Statements)
• Visa photos
• Copy of DS-3032 and X-ray CD

Abigail finished finger printing, initial interview and consul interview.
Abigail received copy of Commission of Filipino Overseas Guidance and Counseling/Registrations procedure and paid Php 247.00 for the Visa delivery.
April 21, 2010 Abigail received the Visa Packet.
May 14, 2010 Abigail went to St. Mary Euphrasia Foundation - Center for Overseas Workers (SMEF-COW) for CFO Guidance & Counseling/Registration with the following:
• Guidance & Counseling Fee of Php 250.00
• Original/Copy of Passport
• Original/Copy of Visa
• CFO Registraion Fee of Php 400.00
Abigail rode jeepney from Sto. Nino to Meycauayan Tollgate, FX taxi from Meycauayan to MRT, MRT from N. Edsa to Araneta Cubao/Cubao, LRT2 from Arenata/Cubao to Katipunan.
Abigail had her passport stamped with registration sticker and counseling certificate.

Tips:
• Schedule your interview as soon as you have the Case Number.
• Take 2-days off work for the medical. Come at SLMCEC by 5:00AM though they'll be open by 8:00AM. Bring 2x2 photos (wag magpabola sa mga nagaalok ng picture). Take a walk from United Nations to SLMCEC about 2 blocks away from Taft Ave (instant excercise kung ayaw mo mataga ni mamang taxi driver). Keep your PDS at all times. Bring some snacks and some books (kakailanganin mo ng pampalipas oras). BE VERY PATIENT!
• Get your birth certificate and certificate of no marriage online.
• Use this electronic DS-156 form from US Department of State website.
• Provide at least recent quarter/year for each I-134 supporting documents.
• Come on-time to US embassy. Don't bring cellphone and the likes (kung wala kang mapagiiwanan or pay 200 pesos). Only answer questions asked. Be confident. PRAY.
• Take Katipunan-Arenata/Cubao underpass exit going to SMEF-COW. Make photocopies of your passport and visa (7 pesos each ang photocopy dito).
• IMPORTANT! Approved visa expires in 6 months.
